These place important obligations on you. You need to read these agreements carefully and understand the meaning of what you are signing. You should answer them truthfully as any omissions will have consequences.
	These cover five areas: Probity: professional obligations Probity: Convictions and disciplinary actions Health: professional obligations Health: regulatory and voluntary procedures Educational contract Read each contract and then click on the column under the Signed heading

	This will bring up a window - for example the Probity: Professional Obligations section. To sign it, click the agreement text at the bottom.

	Once signed, this is the view you will see. You need to repeat this for each contract. Your Educational Supervisor will make sure you have signed this as well as signing the 5th declaration- the educational contract
