You will need to to record a personal learning plan at the commencement of each placement. 

  • Those GPRs who are not sitting nMRCGP: A sensible one to use is the Form 5 "The GMC Personal Learning Plan" taken from the GMC GP appraisal toolkit. Click here to download Form 5. This is different to the PDP from e-Portfolio in that it contains the following:-

    • What development needs have I?

    • How will I address them?

    • Date by which I plan to achieve the development goal

    • Outcome

    • Completed

  • Those GPRs who are ARE sitting nMRCGP: I would suggest you use the PDP form that goes along with the e-Portfolio. This is different to Form 5 in that it contains the following:-

    • Date, 

    • Learning Objectives, 

    • Action Plan, 

    • Time Scale, 

    • How will I know when objective is achieved?, 

    • Achieved

Your trainer may well have other forms that they use within practice to record information, below are a list of examples that may be useful.
