BMJ. This is an excellent site, it is free to use and I would recommend it highly. It has back copies of all articles and an excellent search engine

The Lancet. To get full access to the site, you have to subscribe. You can access much of the site for free however but you do need to register

Bandolier. An excellent evidence based and well researched website givign critcal reviews on many areas. A free site - well worth a visit
